Adoption Application

Please remember that adopting an animal is a serious commitment. Animals may take several weeks to months to settle into their new environment. They cost money and require attention and love. They need veterinary care throughout their lives, including yearly boosters, heartworm and flea/tick preventative medications, etc. If you cannot afford the adoption fee, you may want to reconsider whether you are able to afford the annual care.

Please seriously consider your decision to adopt. Sadly, too many animals are adopted and returned simply because people didn’t think adoption through. We highly suggest that you research what pet is best for you and your family before you adopt.

It is Faithful Hearts’ responsibility to place our animals into an environment compatible with their needs and to ensure their adoption is in the best interest of both the animal and the adopter. Please answer all questions carefully and completely.

*By clicking submit, I agree that I have read the below information carefully and have filled out this application honestly and in its entirety.

I certify that I have never been convicted of animal cruelty, neglect, abuse, or abandonment.

Falsification of any of the above information will be grounds for disallowing the adoption of the pet and possible removal of said pet from my home.

Applicant must be 18 years of age or older.

FHAS reserves the right to refuse any applicant.

I give FHAS permission to investigate, to the fullest extent of their legal capability, the information written above, as well as conduct inquiries with landlords, veterinarians, and related officials.

Please note: Faithful Hearts’ animals MUST be spayed/neutered before they leave our care. If an animal is not of the age of sexual maturity (puppy/kitten), the applicant must sign a sterilization contract agreeing to have that pet altered before the pet reaches the age of sexual maturity.

I realize that FHAS’s goal is to place the animal in a home that FHAS feels is most appropriate, the decision is final, and that the pet is to be returned to Faithful Hearts if an adoption does not work out for any reason, and at any time in the future.

If you are not in agreement with these policies, we cannot place a pet in your family.
